How to register

Registration Forms will be accepted by post, fax, or email. Early Bird registration closes on 1st September 2008 Registration closes on 14th November 2008. Late registrations will be accepted onsite from 8am, 21st November 2008.

The registration fee entitles delegates to the following:

Student Delegate Entitlements

Full time students are eligible for a discounted Conference registration.
Student delegate entitlements are the same as those listed under Full Registration above.

The day registration fee entitles delegates to the following:

The following events are included in the registration fee for Conference delegates (full registration). If you require additional tickets please complete the Social Program section of the Registration Form.

IMPORTANT INFORMATION

Entrance to Conference

Each member of the Conference will receive a name badge on registration. The badge will be your official pass and must be worn to obtain entry to all sessions.

Students

To receive discounted Registrations, full time students will be required to provide a student number from a recognised Australian or international educational institution. Without this or without providing student identification upon request the full registration fee will be charged.

PAYMENT OF FEES

NOTE: Registrations will not be processed or confirmed until payment in full is received.

Within Australia

Payment may be made by cheque or credit card. MasterCard and Visa are the only credit cards accepted at the conference. Please note all transactions by credit card will appear on your statement as payment to Conference by Bayside Health. Cheques should be made payable to Trauma Melbourne 2008

International

Payments from overseas must be made by credit card or bank cheque in Australian Dollars only, drawn on an Australian Bank. Your name and full address should be typed or printed clearly on the back of the cheque. Fees will not be accepted by direct transfer.

Cancellation & Refund Policy

Cancellations must be notified in writing to the Conference Managers.

Registration Fees:

Before 1 October 2008 will incur an A$100 cancellation fee.
Before 1 November 2008 will receive a 50% refund of fees paid.
After 15 November 2008 there will be no refund.

PRIVATE FUNCTIONS

It is requested that you advise the Conference Managers of any private /corporate functions that might be planned outside of the official program. The Conference Managers would be pleased to assist with arrangements for any of these events.

PRIVATE TOURING/HOSPITALITY ARRANGEMENTS

Should you have any specialised group requirements for private touring, transportation or hospitality services other than those organised for the official program, please contact the Conference Manager who would be pleased to assist with arrangements.

INTERNATIONAL DELEGATE VISA INFORMATION

Quarantine

Australia is free from many plant and animal diseases which are prevalent in other countries. Very strict quarantine rules apply to the import of animal and plant material, which cannot be brought into the country without prior application. Animal and plant products are also restricted.

Visa Information

Please note that all travellers to Australia must have a valid visa before boarding their plane. Travellers to Australia cannot apply for a visa on arrival, with the exception of New Zealand passport holders, who will be issued a visa on arrival in Australia.

All international event participants, (other than New Zealander passport holders) should review the following information carefully. There are currently two possible visa options for adult event participants and the appropriate visa to apply for will depend on their country of passport.

IMPORTANT

It is the policy of the Conference office NOT to supply an invitation letter to unpaid or unregistered delegates as it is NOT required and does NOT assist in the visa application process. Upon payment of registration, the delegate will receive a confirmation letter of acceptance of registration and receipt of payment.

ACKNOWLEDGMENTS

Your registration and payment will be acknowledged via email or by post with confirmation of your requirements according to your registration form. Your registration will not be processed or confirmed if payment does not accompany your form.

You will not receive any further publications before the Conference. Your letter of acknowledgment will include any further advice necessary prior to your arrival at the Conference.

INSURANCE

Registration fees do not include insurance of any kind. It is strongly recommended that at the time you register for the conference and book your travel you take out an insurance policy of your choice. The policy should include loss of fees/deposit through cancellation of your participation in the conference, or through cancellation of the conference, loss of international/domestic air fares through cancellation for any reason, loss of tour monies through cancellation for any reason including airline or related services strikes within Australia, failure to utilise tours or pre booked arrangements due to airline delay, Force Majeure or any other reason, medical expenses (including sickness and accident cover), loss or damage to personal property, additional expenses and repatriation should travel arrangements have to be altered. The conference managers cannot take any responsibility for any participant failing to arrange their own insurance. This insurance is to be purchased in your country of origin.

PRIVACY

Australia introduced a Privacy Amendment (Private Sector) Act 2000 in 2001. The Conference Managers comply with such legislation which is designed to protect the right of the individual to privacy of their information. Information collected in respect of proposed participation in any aspect of the Conference will be only used for the purposes of planning and conduct of the Conference and may also be provided to the organising body or to organisers of future events. It is also usual to produce a 'Delegate List' of attendees at the Conference and to include the individual's details in such a list. Consent for publication of the individual's information may be withheld when completing the Registration Form for the Conference. Individuals are also entitled to access the information held by written application to the Conference Managers.
